Position: Aging Life Care Manager/Geriatric Care Manager 

Requirements: 

• Qualified candidates need to hold a master’s degree in related field or a bachelor’s  with 4+ years of experience in Gerontology (AL, SNF, CCRC, Home Care). Music  Therapists, Life Enrichment and Activities Coordinators, Nurses, Social Workers,  Occupational Therapists, and Physical Therapists encouraged to apply! 

Compensation: 70 -85K+ depending on experience

The ElderTree Care Manager works as a team member utilizing their expertise as well as  the expertise and support of the team in order to serve clients so they feel safe, secure, and  empowered. Duties and tasks: 

o Complete comprehensive assessments 

o Design individualized working care plans 

o Coordinate, manage, and oversee care 

o Experience in home care and/or skilled nursing/assisted living communities o Accompany (drive) clients to physician appointments 

o Advocate for client needs in-home or facility 

o Communicate with those involved in client’s care 

o Contribute knowledge base to ElderTree team 

o Support other ElderTree team members 

o Willing to take on-call rotation 

o See clients in their homes, assisted living, and skilled nursing 

o Perform quarterly client file audits, daily charting
