Description: The Associate Care Manager will have the opportunity to learn all aspects of care management through experience and case review with direct mentoring from leadership and Senior Care Managers. Salary: $60K - $75K DOE
What We Need:
Education/Experience requirements:
Candidate should have a minimum of a bachelor’s degree in related field in addition to 3+ years of experience in the field of eldercare or a master’s degree in related field with an internship in the field of aging and be knowledgeable about local resources in the field of eldercare.

Care Management Duties:
Complete comprehensive assessments
Design individualized working care plans, monitor, and update consistently
Complete required documentation daily
Coordinate, manage, and oversee care
Assist in urgent/emergency situations with ElderTree clients
Accompany clients to physician appointments
Advocate for client needs at home or in community
Communicate in a timely manner with those involved in client’s care
Contribute knowledge base to ElderTree team
Support ElderTree team members
Expectations/requirements:
Eligibility for care management certification
Working home computer, printer, scanner, and internet
Ability to write clear and professional notes
Excellent interpersonal and communication skills
Weekly availability Monday through Friday 9am to 6pm
On-call availability as needed per team rotation
Ability to function effectively in a group setting/team player
Self-motivated and able to work effectively with little oversight
Organized and skillful at time management
Flexibility when necessary for new clients and urgent matters
Physical Requirements: ability to push a wheelchair, fold and place a wheelchair into car, and stand by assistance with transfers
