Title: Associate Care Manager (starting position with potential for growth)

This is a full-time position.

Location: ElderTree LLC, based in Northern VA

Education/Experience requirements:

Candidate should have a minimum of a bachelor’s degree in related field plus 3+

years of experience in the field of eldercare or a master’s degree in related field

with an internship in the field of aging.

Purpose:

The Associate Care Manager will have the opportunity to learn all aspects of care

management through experience and case review with direct mentoring from

leadership and Senior Care Managers.

Summary:

The Associate Care Manager works as a team member utilizing their expertise

as well as the expertise and support of the team in order to serve our clients, so

they feel safe, secure, and empowered. We have the same values in common; A

passion for working with older adults and the disabled, a true commitment in

advocating for the best quality of life for our clients as well as a dedication to the

field of aging. We provide assessments, short-term solutions, and long-term care

management.

Team Contribution:

Each team member is asked to contribute their knowledge base to the team to

create the best experience for our clients. Team members work in a collaborative

manor to meeting client’s needs and provide exceptional customer service.

Care Management Duties and tasks:

1. Complete comprehensive assessments

2. Design individualized working care plans, monitor, and update

consistently

3. Complete required documentation daily

4. Coordinate, manage, and oversee care

5. Assist in urgent/emergency situations with ElderTree clients

6. Accompany clients to physician appointments

7. Advocate for client needs at home or in community

8. Communicate in a timely manner with those involved in client’s care

9. Contribute knowledge base to ElderTree team

10. Support ElderTree team members

Expectations/requirements:

1. Eligibility for care management certification

2. Working home computer, printer, scanner, and internet

3. Ability to write clear and professional notes

4. Excellent interpersonal and communication skills

5. Weekly availability Monday through Friday 9am to 6pm

6. On-call availability as needed per team rotation

7. Ability to function effectively in a group setting/team player

8. Self Motivated able to work effectively with little oversight

9. Organized and skillful at time management

10. Flexibility when necessary for new clients and urgent matters
