Care Manager, Contract Position:

Do you want to:

  • Work independently and set your own schedule and hours?

  • Determine the size of your caseload?

  • Work throughout the continuum of care visiting clients in their own homes, within senior living communities, nursing facilities, and hospitals?  

  • Work closely with attorneys, home care agencies, and allied professionals?

  • Combine and utilize your clinical skills with case management?

  • Handle interesting and stimulating situations working with clients and family members from many backgrounds and on broad issues?

 Seabury Care Managers are widely recognized as a group of dedicated, compassionate and experienced professionals who are all members of the Aging Life Care Association (ALCA). Our care managers provide assessments, guidance and support for older adults that focuses on maintaining and enriching quality of life.   

Seabury Resources for Aging is seeking a Master of Social Work or Registered Nurse to provide care management to older adults in the DC Metro area. This is a great opportunity which offers the independence of private practice while having the benefits of an experienced team of professional care managers. Candidates must have a minimum of 5 years post graduate experience, strong clinical skills, excellent oral and written communication skills, possess strong organizational and problem solving skills, good record keeping, and the ability to set priorities and meet goals.  Must have equipment and ability to maintain electronic records.  Use of personal car is required.  Appropriate licensure, malpractice insurance and car insurance required.  Experience in health care, medical social work or case management preferred.  Bilingual in English and Spanish and experience in mental health a plus.
