ElderTree Care Management founded in 2004 is a leader in Care Management in the DC Metro Area. We are looking for a few additional Team Members to serve as Care Managers and eldercare advocates in Northern Virginia.

Care Manager Job Description

Title:
Care Manager 20 hours part-time /35 hours full-time

Purpose:
The ElderTree Aging Life Care Manager provides client-centered care to older adults or those with disabilities ensuring the highest quality of life on a short or long-term basis. To achieve this client goal we: 1. Assess & monitor, 2. Plan & problem solve, 3. Educate & advocate.

Summary:
The ElderTree Aging Life Care Manager works as a team member utilizing their expertise as well as the expertise and support of the team in order to serve clients so they feel safe, secure, and empowered. We all have the same values in common; A passion for working with elders and the disabled, a true commitment to advocating for the best quality of life for our clients as well as a dedication to the field of aging. We provide assessments, short-term solutions, and long-term care management.

Duties and tasks:

  1. Complete comprehensive or placement assessments

  2. Design individualized working care plans

  3. Complete required documentation in a timely manner

  4. Coordinate, manage, and oversee care

  5. Monitor working plans of care

  6. Assist in emergency situations

  7. Accompany clients to physician appointments

  8. Advocate client needs whether in-home or facility

  9. Communicate with those involved in client’s care

  10. Contribute knowledge base to ElderTree team

  11. Support ElderTree team members

Expectations/requirements:

  1. Professional computer skills and knowledge

  2. Convenient access to computer, printer, and internet

  3. Ability to write clear and professional notes

  4. Excellent interpersonal and communication skills

  5. Weekly availability Monday through Friday 9am to 5pm

  6. On-call availability team rotation one week

  7. Possess qualifications for certification

  8. Ability to work in a group setting/team player
