Aging Well Eldercare is one of the area's longest-practicing Aging Life Care Management services, celebrating our 35th anniversary this year. As professionals dedicated to Aging Life Care Management, we offer knowledge, experience, and compassion to help clients and their families with the challenges of aging and disability. Our team consists of twelve Aging Life Care Managers (nurses, gerontologists, and social workers) who work well together and support each other. Our dedicated office staff supports the practice and aging life care management team.
We are looking for candidates who have the following qualifications & experience:
Active LMSW/LCSW, Registered Nurse, OR Physical Therapist licensure
CCM certification a plus
Experience in geriatrics, dementia, home health, mental health, or hospice care is required
You need to be a self-starter, able to work independently and communicate well with clients, families, and other professionals
This position requires making in-person visits to our clients in the DC Metro area, including Frederick
Duties & Responsibilities:
Performs the initial assessment for clients.
Accompanies clients to medical and other appointments.
Makes regular client visits to implement care plans and monitor status. The frequency of visits (weekly, biweekly, monthly) is to be determined in consultation with the family and reviewed with the leadership team as needed.
Writes client care plans and submits them to the leadership team for review.
Prepares monthly written client summaries.
Conducts ongoing communication with family and other service providers (includes supportive counseling for client and family). Communicates regularly with family members by phone or email, after each visit, or less often as preferred by the family.
Consults with Director and other staff regarding urgent issues such as a change in health status; the need for increased services; crisis intervention; problems with communication with family members or other health care providers; and ethical issues.
Develops working relationships with leaders in the community by participating in professional networking activities, and sharing information about our services
Provides occasional week-end on-call support to respond to urgent client needs or emergencies
Schedule:
20 to 30 hours per week, Monday to Friday (flexible)
Occasional weekend on-call

Salaried, commensurate with experience
