Overview of Capitol Hill Village: Founded in 2007, Capitol Hill Village (CHV) is a non-profit membership organization, with over 400 members and 250 volunteers. CHV is a member of the DC Villages Collaborative and one of approximately 300 Villages around the country. Based on the concept of neighbors helping neighbors, CHV provides members with social, educational, cultural opportunities, as well as support services including transportation, home maintenance and other home support services performed by CHV’s vetted volunteers or vendors. CHV is a volunteer-first organization, meaning that volunteers are involved in all aspects of our work and partner regularly with staff. Additionally, CHV’s care services team provides individual support, resources and referrals to support aging-in-place, help for members to navigate health care, insurance, social services, housing, and other issues. CHV’s goal is to help members live life on their own terms and remain in the community and in their homes as they grow older. CHV has programs and services for members across the spectrum of health, mental acuity, and economic means. We are committed to diversity in our staff and membership.

Care Services Specialist Position Description: Capitol Hill Village seeks a fulltime Care Services Specialist (CSS) to help with care coordination and clinical case management, providing information and referrals, and engaging members to increase social and emotional wellness while aging-in-place. CHV staff are expected to seek proactive engagement with members. This position would include approximately 85% case management, and 15% programmatic support of care services activities and organizational initiatives. CHV has a team centered approach and staff are expected to embrace opportunities to work with others, collaborate and support colleagues in their efforts, and provide coverage for peers as needed. 

Responsibilities include:

  • Performing initial and ongoing assessments of members in their homes, or virtually
  • Considering appropriate interventions, resources, and referrals for members according to assessment findings
  • Guiding members, their families, and other stakeholders through crucial decision-making processes using strength-based techniques
  • Serving as a liaison among members, community agencies, and other providers, and directly oversee the coordination of care to support the member as he/she/they age in the community
  • Researching and developing partnerships with community agencies and organizations
  • Maintaining and updating CHV's in-house resource directory
  • Appropriately documenting all member interactions for cohesive case coordination with services team
  • Assisting in data collection and analysis relevant to program evaluation
  • As requested, assisting with organization-wide programming and initiatives
  • Answering phone and email inquiries
  • Collaborating with team to respond to member and organizational needs as they change
  • Engaging with a flexible mindset, positive attitude and strengths-based approach with the goal of identifying concerns, finding relevant information and resources, evaluating efficacy of ongoing process, creatively engaging stakeholders, and identifying solutions that reflect member-centered solutions
  • Supporting anti-racism and inclusion efforts at the staff and organizational level

Care Services Specialist Position Requirements

  • MSW degree or similar, with a focus on individual case management
  • Post-graduate licensure in DC with LICSW preferred, or written agreement about timing of DC licensure
  • Demonstrated interest in working with older adults, including as part of multi-generational families
  • Experience working with web-based programs and basic office software, including Zoom, Microsoft Teams. MS Word, Excel, and PowerPoint and willingness to learn and utilize CHV’s internal database
  • Demonstrated interest in working across gender, sexuality, racial, abilities, cultural and class lines required, and experience with marginalized communities preferred
  • 2 years' experience in community-based social service or not-for-profit preferred
  • Fulltime availability (office hours are 9am-5pm); occasional evening or weekend work
  • Strong written and verbal communication skills
  • Minimum three days per week in office, possible for two days per week remote
  • Capacity to do home and other off-site visits (a car is not required for this position)

Salary and Benefits

  • Salary range starts at $70k, depending on licensure and experience
  • Weekly 1-hour individual supervision with Director of Care Services and weekly case consultation with Care Services team
  • Health insurance and disability insurance

  • 24 days personal time off plus most federal holidays
