Summary of duties:

Coordinates medical needs of the Residents with physicians, Resident Wellness Director and family members. Administers medications. Observes, documents and reports significant changes in Residents' condition, recommending changes in care needs. Supervises Wellness LifeStories team members, determines shift assignments, provides instruction to Wellness LifeStories team members and ensures care is provided as instructed. Assists with IntegraCare and State required assessments. Supports and conducts programming for residents as needed. Provides oversight of Wellness team in conducting programming in collaboration with the LifeStories Director.
