Position:  Client Care Specialist/Sales Consultant (Part-time)

Reports to:  President/Owner


Hours: 8:30am-5pm (three weekdays per week; 24/hours per week), 

Mission Statement: Enhancing the lives of aging adults and their families.


Primary Responsibilities:

Care Consultations:

Conduct initial care consultations with seniors and their families who are interested in home care services for their loved one; close ratio goal: 80 percent. 

Create Companion Plan of Care and ensure ClearCare Telephony tasks match it. 

Input new client information into database.

Make Director of Operations immediately aware of status on care consultations; document in Google calendar.

Continue to follow up on care consultations that do not close.  Document in ClearCare.

Update Care Consultation spreadsheet.

Service Inquiries:


Assist with taking service inquiries, with goal of setting up care consultations.

If do not convert service inquiry into care consultation, create task so that Director of Operations can follow up.

Submit completed Service Inquiries that Do Not Convert to Care Consultations spreadsheet to Scheduling Coordinator by 5pm each Friday.


Quality Assurance:

Conduct introduction between CG and client on first day of service.

Conduct quality assurance home visits for Companion Care clients, every 30 days.  Conduct at least 10

  visits per week.

On QA visits, a) complete QA form b) review Client Journal for accuracy c) ensure clients remain in

  Scope of Practice d) update Companion Plan of Care, when appropriate and ensure ClearCare

  Telephony Tasks match it e) suggest relevant senior-related resources e) look for opportunities to

  add client hours when appropriate. 

Visit clients in rehab; attend discharge meeting; develop Plan of Care upon release from rehab or



Visit one client-related RPN (facility, rehab, home health, etc.) weekly.  Establish ongoing relationship

  with RPN, with goal of client referrals.

Ensure clients complete four postcards, monthly.

Attend monthly GROWS and/or DC Resource Group meetings. 



Conduct new-hire employee training (one-hour initial review), bimonthly.

Secondary Responsibilities:


Submit Client Care Specialist Scorecard by 7th of each month.

Attend periodic training and other company events.

Assist with other duties and special projects, as assigned.

Skills Required:

Must possess strong interpersonal skills, be a good listener, and be comfortable with sales and marketing (i.e., signing on new clients).

Must have excellent written and oral skills.

Must have an understanding of the geriatric population.

Must be adept at problem solving, including being able to identify issues and resolve problems in a timely manner.

Must be able to prioritize and plan work activities efficiently.

Must be organized, accurate, thorough, and able to monitor work for quality.

Must be able to work independently.

Must be a team player.
