Lifematters is currently seeking a talented and experienced social worker, RN or senior home care professional to join our management team as our Director of Flexcare Services. This qualified professional is responsible for the overall management of the organization's satellite sites, including managing staff, serving as a point of contact for the senior communities where the sites are located, and facilitating communication between site personnel and the main offices. This position is a member of the Operations/Management Team.


  • Manage staff at assigned satellite locations
    • Visit weekly
    • Review client services
    • Prepare performance appraisals for staff and review with staff
    • Facilitate communication between satellite office and main office
    • Assist with caregiver/client scheduling 
    • Equipment review
  • Liaison with Community Administrators
    • Resolve issues that may arise with caregiver and clients
    • Communicate community policies and make sure our staff adheres to those policies
    • Assist in developing plans to keep clients safe and independent in their homes
  • Meet with potential, existing and past clients
    • Resolve issues that may arise between staff
    • Propose and Implement care that meets client's needs and budget
    • Make in-home visits and evaluate client satisfaction and caregiver performance
    • Take care history, explain and execute new client paperwork
    • Resolve billing issues
  • Additional duties include:
    • Represent Lifematters publicly at meetings/health fairs, and other activities as required/directed.
    • Keep the Client Services and Clinical teams informed of issues with clients/families, caregivers, and new policies in the communities.
    • Make sure client/community issues are documented and routed to appropriate team members.
    • Insure that new client paperwork is complete and entered into the company data base.
    • Attend management meetings as scheduled.
    • Provide written and verbal reports on both routine and special projects, including monthly and annual reports.


  • Bachelor's degree required. Graduate degree in related field with home care background encouraged.
  • At least three to five years' experience in healthcare staffing or other related health care field.
  • Strong computer skills and experience working with data bases and inputting data.
  • Experience executing contracts and developing care plans.
  • Strong interpersonal skills and ability to communicate with a wide variety of individuals in a tactful, professional, and compassionate manner. 
  • Ability to problem solve and troubleshoot issues with clients, caregivers, and community personnel, on location.
  • Must have the ability to work independently without constant supervision. This requires a high level of productivity and using time efficiently to plan out the work appropriately.
  • Able to use necessary equipment and supplies, as well as manage routine office tasks. 
  • Strong Microsoft Office, data base and technology skills including ability to maximize use of a smart phone while at site locations
  • Must have reliable transportation and a flexible schedule.
