POSITION OVERVIEW
The position is primarily responsible for the clinical management of a caseload of patients facing end of life and working with the interdisciplinary team to create a plan of care that meets the patient/family needs and goals. This position is also eligible for an annual team performance bonus.
JOB DUTIES
Assumes primary case management responsibility for a patient/family caseload that includes the initial assessment, continued assessment, planning, implementing and evaluating all phases of nursing care.
When needed, assesses patient needs and admission for service and follows protocol in admitting patients who meet eligibility criteria.
Re-evaluates the patient’s clinical needs at least every 14 days and is able to articulate continual eligibility, including documenting appropriate subjective and objective criteria.
Initiates, implements, coordinates and documents the patient’s plan of care with the IDT in accordance with nursing needs specified in the approved plan of care.
Delivers or supervises skilled nursing services using evidence-based knowledge regarding end-of-life care.
Documents all clinical nursing visits, medication additions and changes, care plans, and additional documentation per protocol within 24 hours of provided service.
JOB REQUIREMENTS
Minimum of a bachelor’s degree.
At least two years of administrative experience, preferably hospice or discharge planning.
Is able to speak articulately about end-of-life issues and able to compassionately and calmly respond to clients who under duress.
Proficient in Word, Excel and able to master documentation software competently enough to assist others in its use.
