Licensed Clinical Social Workers contribute to the CounterPoint Mission by working with the team to provide excellent clinical and supportive care to patients and facility partners.
SCOPE OF RESPONSIBILITES:
Licensed Clinical Social Workers have the following responsibilities and duties:
Responsible for providing behavioral health services to patients
Scope of services includes behavioral health evaluations and treatments
Assess, diagnose, and treat adults experiencing symptoms of a behavioral health disorders
Recommend non-pharmacologic interventions for psychiatric and cognitive disorders
Collaborate with primary care, psychiatry staff, social workers, or other clinical team members to coordinate patient care
Responsible for daily and accurate completion of medical records, including assessment, treatment plan, progress notes, outcome measures, and billing entries
Design/participate in the development of individualized treatment plans, incorporating best practices into treatment interventions
Collaborate with responsible parties, guardians, facility staff, primary care, and others on patient’s condition and treatment when clinically appropriate
Prepare and submit assessments, progress notes, reports, and summaries to entities/agencies (funders, insurance companies, etc.) as required
Return calls and emails promptly (within 24 hours of receipt or sooner if clinically necessary)
Responsible for making independent decisions based on protocols developed utilizing appropriate clinical guidelines for treatment interventions.
Responsible for maintaining customer relations with all facility staff and patients
Records all services performed for accurate billing for services provided in compliance with all local, state, and federal laws, as well as payor rules and regulations.
Submit billing documents daily.
Looks for ways to improve and promote quality.
Provides education to patients, family members, and facility staff, if requested.
Follow ethical and legal policies, procedures, and standards
QUALIFICATIONS:
Licensed Clinical Social Worker should have the following skills, education, and experience:
Licensed clinical social worker in state of practice
Master’s degree in clinical social work
3+ years of experience preferred
Experience in delivering high-quality, client-centered care
Ability to effectively communicate and interact with individuals involved in all aspects
Proficient computer skills
Ability to work independently effectively
Ability to read, analyze and interpret general and healthcare documents
Ability to write routine clinical notes, prescriptions, and correspondence
Excellent organizational skills, including the ability to work productively and make independent decisions.
REPORTING:
He or she will report directly to the Vice President of Behavioral Health Services.
