The Full-Time Nurse Assessor  will be responsible for the management of all aspects of the State assessment program. Ensure that proper support and expertise are provided to age-in-place clients through the following processes: comprehensive assessments, planning, implementation and overall evaluation of the specific client's needs.  


  • Conduct daily age-in-place assessments and determine appropriate service recommendations
  • Provide support and supervision to the Assessment team
  • Develop a supportive and effective relationships with clients
  • Conduct follow up assessments and recommend any changes to services required
  • Work closely with the QC Director and Training Director in developing scalable policies and procedures for all functions regarding the Assessment division
  • Coordinate and maintain all Assessor training and certification programs
  • Develops, implements and make improvements to the coordination and tracking of team activities
  • Provide assistance with policies and procedures and ensure full compliance by the Assessors
  • Work closely with the functional management team to ensure smooth operations and coordination between the functions
  • Organize and maintain files and records of a confidential, complex or otherwise sensitive nature according to State and Federal regulation
  • Maintain accurate up-to-date information for each individual served and maintain other files as necessary for the success of the organization
  • Participate in meetings and training sessions that offer learning opportunities and that promote and enhance skills and professional development

  • Bachelor’s degree preferred
  • Current valid RN License in the state of Maryland or compact state required
  • Minimum of 3 year of experience working with the aging population
  • Ability to identify barriers to aging in place to make appropriate recommendations a plus
  • Assessment skills required
  • Need to be flexible and have demonstrated ability to work cooperatively with staff at all levels
  • Must have strong computer skills and have experience with record management systems
  • Must be detailed oriented and resourceful
  • Demonstrated ability to take initiative
  • Effective communicator, both orally and written
  • Must be able to successfully pass background investigation.
