The Option Group is seeking a Part-time Care Manager in the Philly Pennsylvania area to join our awesome team in providing excellent care management services to our clients.  LCSW or MSW degree is preferred along with case management and/or case coordination experience.  Salary is based on qualification and experience. 

Candidates must possess a good working knowledge of dementia and knowledge of community resources. The ideal candidate is self-motivated, proactive, organized, and has excellent problem-solving skills and communication skills.
