Our company provides a wide range of individualized services to the elderly, disabled and their families. The Geriatric Care Manager (GCM) is trained and experienced in any of several related fields, including gerontology, social work, nursing or psychology. The Case Manager is a professional with specialized skills in the psychological, emotional and social aspects of geriatric client care, with a working knowledge of community and social resources. Geriatric care management is a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges and/or cognitive impairment. Working with families, the Professional Geriatric Care Manager’s expertise provides the answers to a variety of issues at times of uncertainty.
Hourly Compensation (between $25-35 per hour) based on experience and credentials.

Position Description: The GCM assists in a variety of areas including:
 Medical management – accompany (drive) clients to physician’s appointments, facilitating communication between doctor, client, and family, and if appropriate, monitoring client’s adherence to medical orders and instructions
ï‚· Performing comprehensive psychosocial assessments to determine client service needs; implement Plan of Care
 Housing – helping families evaluate and select appropriate level of housing or residential options in the community
 Safety and security – monitoring client at home; recommending technologies to add to security or safety; observing changes and potential risks of exploitation or abuse
 Communication – keeping family members and professionals informed as to the well-being and changing needs of the client
ï‚· Acting as a liaison to families at a distance
ï‚· Visiting clients on a regular, routine basis to make sure they are safe, doing well, eating properly, and taking needed medications
ï‚· Referral to community resources
ï‚· Post-hospital discharge planning
ï‚· Provide on-going case management to individuals so they can maintain their independence
ï‚· Maintain client files and daily documentation
The Ideal Candidate:
ï‚· Case Management experience in a multidisciplinary clinical setting (i.e., hospital, long term care community, case management organization).
 Master’s degree in field of Geriatrics; LCSW or RN preferred
ï‚· Two years of experience in geriatrics, care management or related disciplines required.
ï‚· Ability to interface effectively and professionally with client support network, including family, physicians, attorneys, etc.
ï‚· An entrepreneurial spirit, drive, and goal-oriented, strong work ethic
ï‚· High energy and a positive attitude, with the ability to excel under pressure
ï‚· Must have own transportation and be able to drive to meet clients and/or their families
ï‚· Strong written and verbal communication skills.
ï‚· Ability to prioritize, plan, flex and manage a very busy work environment.
