THE WORK:
The Full-Time RN/Case Manager will be responsiblefor maintaining a positive partnership with DwellAssured OT Assessors and assigned clients throughout the assigned geographical area. As a trusted advisor, you will use your expertise to manage client accounts and facilitate the execution of the client’s aging in place plan. You will also monitor the quality of the client experience with DwellAssured Service Providers.
Responsible for ensuring that proper support and expertise are provided to age-in-place clients through the following processes: comprehensive assessments, planning, implementation and overall evaluation of the specific client's needs. Will be required to conduct in-person client meetings as scheduled and/or as required throughout the year to ensure success with the age in place assessment process.
THE RESPONSIBILITIES:
Conduct initial and follow-up assessments and recommend any changes to services required
Participate in all assessor training and certification programs
Conduct follow up calls with clients to reinforce the partnership that exist between clients, Service Providers and OT Assessors
Build a trusting relationship by establishing a friendly and reassuring rapport with clients
Push forward in the implementation of the aging in place plan
Manage client profiles (Ex. personal status changes, identify new requirements)
Determine if the most recent client service provider requests have been performed
Perform quality control evaluations with Service Providers
Schedule the next three highest priority service requirements
Schedule client assessments with Assessors
Manage Client Assessment Direct Mail
Conduct in person (travel) assessments annually or more frequently as needed to determine appropriate service recommendations identify barriers to aging in place
Answer/manage incoming calls from clients and ensure continuity of services

THE REQUIREMENTS:
Bachelor’s degree preferred, preferably in the Health or Human Services field
Current valid RN License in the state of Maryland or compact state required
Minimum of 3 years’ experience working with the aging population
Ability to identify barriers to aging in place to make appropriate recommendations a plus
Assessment skills required
Need to be flexible and have demonstrated ability to work cooperatively with staff at all levels
Must have strong computer skills and have experience with record management systems
