Description of Home Team Program:
Social engagement and support systems are important for everyone, but especially older adults. As people age, it becomes more difficult to remain connected to the community. The likelihood of isolation increases due to a variety of life-changing factors. Isolation can lead to loneliness, depression, fear, anxiety, malnutrition, rapid health decline, and other mental disorders. Fortunately, with the help of generous people willing to donate their time, energy and talents, we are able to enhance the quality of lives of those individuals at risk. Through the Baltimore County Home Team, volunteers provide friendly in-home and telephone visits to eligible older adults who lack social supports and are at risk of social isolation.

Persons assigned to a home team volunteer are Baltimore County residents age 60 or over who have been referred by a human services agency. Volunteers are asked to make a six-month commitment of approximately one hour a week and are matched with people in their neighborhoods. This program, which operates through the Baltimore County Department of Aging (BCDA), provides ongoing training and recognition to volunteers. The program reflects a caring sense of community, providing reassurance and security to older persons wishing to remain in their own homes and at the same time providing satisfaction to volunteers.

Position Description:
The Home Team Field Coordinator is responsible for the program coordination of an assigned region in Baltimore County. This includes the facilitation and maintenance of all volunteers and clients placements.

Volunteer Recruitment and Placements
Responsible for the vetting and placement of volunteers in assigned region. This includes developing a targeted recruitment plan, outreach and community presentations, marketing program materials, reviewing and tracking applications, gathering documentation, conducting interviews, coordinating background checks, and facilitating training/orientation and placements as appropriate.

Client Intake and Placements
Responsible for facilitating the intake process of clients in assigned region. This includes developing a targeted outreach plan, outreach and community presentations, marketing program material, reviewing client referrals, identifying potential clients, gathering all necessary paperwork, conducting in-home eligibility assessments, and facilitating placements with assigned volunteers, as appropriate.

Coordination of Placements
  • Track and monitor volunteers and client placements for assigned region, ensuring program compliance, continued eligibility, on-going reporting and support as needed.
  • Work with office assistant to maintain individual case files and all required documentation to ensure the accuracy of the program database, rosters and other reports.
  • Develop and maintain working relationships with client case managers, referring agents, building managers, family members and community associations (as appropriate).
  • Provide general coordination of region for all program participants.

Skills Sets necessary for satisfactory completion of duties:
  • Associate Degree in human services related field (preferred)
  • 2 years demonstrated experience in volunteer program coordination
  • Self-starter
  • Case management related experience or training
  • Experience in conducting assessments related to seniors or a similar population in the home setting.
Computer skills include:
  • Microsoft Office, the ability to type, use the computer and manipulate search engines with ease.
  • General office skills
  • Compassion/patience with seniors
  • Excellent customer service & networking skills
  • Proficiency in oral and written correspondence
  • Fluent in English and experienced in public speaking
  • Strong critical thinking skills and networking/partnership building skills
  • Medical Examination and Employment Background Investigation
  • Applicant's selected for an appointment to a position in Baltimore County must successfully complete a physical examination, drug screen and an employment background investigation, including, but not limited to a criminal background, education and fingerprint check.
  • Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.
  • Ability to stand, walk, lift up to 25 lbs. periodically.

  • Sick Leave – Monthly sick leave earnings prorated based on the number of hours worked per week. No maximum on accrual.
  • Vacation Leave – Monthly vacation leave earnings prorated based on the number of hours worked per week. Earnings increase after five, 10 and 15 years of service generally.
  • Holidays –Part-time, non-merit or non-classified employees working 29 hours per week or less will receive two paid holidays per calendar year. These holidays are the Christmas holiday and the Independence Day holiday.
