  • Completes social histories upon residency to Residential Care (RC/Assisted Living).
  • Complete Service Plan requirements for all residents within prescribed regulatory  time frames.
  • Attends daily meetings of the Interdisciplinary Team to be apprised of and report on significant issues and developments for RC/IL residents.
  • Work collaboratively with the interdisciplinary team in developing Care Plans and discharge plans for the RC residents.
  • Coordinate with staff, residents, and their responsible parties, all permanent transfers from assisted living and independent living to the Healthcare Center(HCC)
  • Provide psycho-social support to RC and IL residents, their family members/significant others, and/or responsible parties.
  • Act as liaison between Roland Park Place and area hospital discharge planners when:
  • RPP residents are inpatients in order to facilitate appropriate discharge plans (i.e., to HCC or apartment).
  • For potential per diem admissions to the HCC.
  • Ensures all required documentation is accurate, timely and complete.
  • Conducts new residency medical approvals.
  • Facilitates Care Giver Support Group
  • Performs other duties as assigned.

EDUCATION:    Bachelor’s Degree in human services field; BSW and LBSW licensure.  MSW preferred.

EXPERIENCE: Two (2) years of related experience in human services field; at least one (1) year experience working with the elderly required; Assisted Living experience preferred.  Knowledge of geriatric population and dementia.
