Smith Life Community, a premier 558-bed long-term care facility with a 93 bed sub-acute unit located in Rockville, Maryland is seeking a Case Manager to join our sub-acute/short-stay rehabilitation.

Located on a 34-acre campus in suburban Rockville, MD, our system provides a continuum of care encompassing long-term care at the Hebrew Home of Greater Washington, assisted living at Landow House, memory care assisted living at Cohen-Rosen house, independent living at Ring and Revitz House, and the Hirsh Health Center, an outpatient health clinic. The Case Manager will be responsible for conducting initial and on-going bio-psychosocial assessments, cognitive and mood interviews for each resident. Develop care plans and multi-disciplinary discharge plan based on assessments. Complete social work sections of MDS. Gather information and assist residents and families with all aspects of discharge planning including making referrals for medical equipment and community services.

A qualified candidate will have a minimum of one to two-years experience as a discharge planner or a case manager in a health care setting—hospital experience preferred. A bachelor’s degree in social work or related field is required; a master’s degree in social work is preferred.
