Transitions Coordinator for JSSA Hospice

General Overview:
Our Hospice Program has an immediate full-time opening for a Transitions Program Coordinator. The Transitions Program is a social support, pre/post-hospice program for seriously ill individuals and their families. This position works closely with our Hospice staff and Volunteer Coordinators to ensure continuity of care and service. Predominantly field-based, Transition Coordinators provide visits to clients in their private residences, nursing homes, and assisted living residences and coordinate volunteer support to them.

Position Qualifications:
Qualified candidates will have a bachelor’s degree in nursing, social work, counseling or a related field and five years of experience; RN preferred. Ideal candidates must demonstrate the ability to provide quality care in a fast-paced, growing and supportive environment with an emphasis on collaboration and providing exceptional customer service. Excellent communication and organizational skills, and attention to detail are required. Qualified candidates will have proficient skills with MS Office (Word, Excel, Outlook) and familiarity with electronic medical records. Reliable and independent transportation is required.
