Our goal is to allow patients to live out the remainder of their days as comfortably as possible, while also providing support to their families. Learn more about our mission and philosophy.

    Services offered through BridgingLife include:

BridgingLife offers hospice, palliative care and grief support servicesHome-based and Inpatient Hospice Care:

To make the most of the care and services offered by BridgingLife, a patient may require different intensities (levels) of care during the course of his or her illness.

The Medicare/ Medicaid Hospice Benefit (and many private insurance companies) provides four levels of care to meet the patient’s clinical care needs, including routine home hospice care, general inpatient care, continuous home care and inpatient respite care.

Regardless of the hospice level of care, you and your loved ones have access to hospice staff 24 hours a day, seven days a week. After-hours telephone support and in-person visits are available to handle anticipated and unplanned changes in the patient’s condition.

    Grief Support and Counseling:

Our bereavement team offers up to 13 months of counseling sessions for hospice patients’ families. These sessions help families realize that the feelings they are experiencing are normal and allow them to work through and understand those feelings. In addition, our bereavement counselors oversee our grief support groups, open to all in the community, where thoughts and feelings of those who have experienced loss can be shared with others.
