No post hospital case management that have most of them return to the hospital due to falls, recurring illness, etc.  Quite a few have Medicaid that may not cover the in home expenses.  Is there a process to be done that patients are prepared prior to release?

As a dementia specialist, I wanted to comment as to how important it is to include the person living with dementia in the process of the move. Dropping a person off on the day of the move is never helpful. Change and loss is difficult for every person, especially a person living with dementia.  By including the person in preparation for the move – discussing the positives of the new residence, letting them choose belongings, decorate the room, ask questions, will help each person adjust much more easily.
