The Option Group is looking for a Care Manager with a master’s in social work and at least three years of experience, preferably in the hospital setting. The geographic area is near Washington, DC, including central Maryland, Rockville, and the Gaithersburg areas.

The care manager is responsible for managing clients' clinical and medical care, including vulnerable adults and elderly populations. This position is also expected to support the overall operation of The Option Group when needed.

The Care Manager conducts home visits, phone calls, virtual calls, and regular in-person visits with clients to gather information, assess their condition and living situation, and make recommendations. 

Connect evaluation and clinical assessments to discern the situation and make recommendations. 

Develop appropriate service plans with individuals, families, and service providers in accordance with regulatory agency policies.

Advocate for the client to get services, entitlements, or the best care scenario for the client.

Connect clients, families, and/or representatives to appropriate services and resources.

Accurate document client files and maintain compliance with local health departments and the Department of Aging, and enrolling individuals in qualifying programs. 

Knowledge of resources includes Medicare, Medicaid, insurance, local/regional facilities, services, practices, and community resources. 

Strong attention to detail and organizational skills, with the ability to prioritize and complete multiple work assignments on time. 

Commits to exceeding expectations and delivers the highest level of customer service. 

Good working knowledge of dementia, its behaviors, challenges, and clinical pathways.
