12:49:55     From  steve jefferys : I was with my dad for five days at a hospital ICU in Florida. I went home to MD and my brother was coming from PA to take my place overseeing his care. There was one day that no brother was there at the hospital and that’s the day the hospital sent him to a nursing facility, instead of a step-down unit which he should have gone to. He was at the nursing home with limited oversight from staff for the first four hours. He died a few days later.

12:54:03     From  Isabelle Melese-d'Hospital   to   All panelists : I feel you, Steve, that almost happened to my dad back in 2013 where he was slipping through the cracks even with a constant stream of family checking on him.  It’s why people need patient advocates like myself and my colleagues, so that we can help out when the family can’t be there.  My condolences.  Isabelle Melese-d’Hospital, Ph.D.  Tournesol Services, LLC www.tournesolservices.com

12:56:01     From  Victoria Hathaway : YES to pushing back on the docs and hospitals! My dad was not in relationship with a PCP, was spiraling down into dementia, and his sentinel event was behind the wheel, first responders took him to the nearest ER.  They treated him like a GOMER (Get Out of My Emergency Room) and called me to come pick him up.  I pushed back HARD and demanded they admit him, diagnose, etc. TEN DAYS later he was discharged direct to memory care, ALZ diagnosis, full work up, great care and discharge support.  Had he left the ER that day we’d have had zero leverage to get him diagnosed and cared for.  Families are lambs to the slaughter in this regard.  Thanks for this coaching, push back!

12:57:38     From  Christine Dolan : FANTASTIC panel and information - thank you!!!

12:58:25     From  Linda Chaletzky : I was thinking of the “early discharge” when a patient is sent home prior to the number  of required nights in the hospital for rehab or other medical placement to be covered by insurance. Sometimes if you know that there is a bank of days doctors can draw on it gives you more leverage in your NO

13:00:34     From  Cynthia Hajjar : Great topic and presenters! Thank you!

13:02:08     From  Margi Helsel-Arnold   to   All panelists : Linda-Hi—right now they have still waived the three night requirement for Medicare skilled nursing home care-SW’s in the hospital still get the push to discharge someone if the doctor writes the order—sometimes family can talk with doctor to ask for extra tests but usually  my experience they will discharge and not want the person to remain in the hospital for many days unless there is justification—Margi

13:04:55     From  Linda Chaletzky : Hi Margi-Thank you for your response. I have clients who often find themselves in the hospital and although I don’t get involved in the decision making, I do like to understand what current policy is. So thank you.
