To help with this, our nurse will work individually with you to develop a personalized long-term care plan designed to manage all of your home care needs. Once the plan is established, a Care Team Leader will be assigned to you that can coordinate care and any changes in the level of care you or your loved one needs. A Care Team Leader is available 24/7 so that every urgent need is handled by a real-life person, not an answering machine.
