Our care managers guide older adults and their families through the process of planning for long-term needs and respond to urgent situations.  They provide counsel to families on key issues, advice on next steps, and connection to trusted  elder law attorneys, home care providers, senior living communities, physicians, rehabilitation centers, mental health providers, financial planners, and others to promote a positive aging experience.

Care Management services include:

• In-home & virtual assessments, care planning & service coordination
• Objective & informed recommendations for in-home services & residential care
• Advocacy & coordination with physicians, attorneys, insurance providers & care facilities

Seabury’s Life Enrichment Services support older adults in activities that foster conversation and social interaction. This program addresses isolation and re-engages clients in their hobbies and passions. 

Life Enrichment Specialists are experienced and educated companions for:

• Outings such as walks, lunch, shopping & visits to beauty/barber salons
• Cultural & social activities & worship services 
• In-home games, entertainment & conversation
• Personal appointments including medical, dental & therapy appointments
